Launch Physical Therapy 6th Anniversary Scavenger Hunt Contest Rules and Terms & Conditions

By participating in the Launch Physical Therapy & Sports Performance Center 6th Anniversary Scavenger Hunt (“Contest”), you agree to the following rules and terms & conditions:
1. Eligibility
- Participants must be current, past, or new patients of Launch Physical Therapy & Sports Performance Center.
- Employees (current or past) of Launch Physical Therapy & Sports Performance Center are NOT eligible to participate in this Contest.
- Contest is open to legal residents of the United States who are at least 18 years of age at the time of entry.
2. Entry Period
- The Contest will run from September 16, 2024, to September 20, 2024.
- Entries must be submitted by 11:59 PM MST on September 20, 2024.
3. How to Enter
- Participants will receive a daily email with a riddle. Solving the riddle will guide them to a hidden Launch logo on our website.
- Each correctly solved riddle and successful finding of the hidden logo will count as an entry into the drawing for prizes.
- Additional entries can be earned by participating in daily challenges and trivia on our Facebook and Instagram pages during the contest period.
- There is no limit to the number of times participants can enter the contest.
4. Prizes
- Participants have the chance to win one of three prestige baskets or the grand prize. Details on prizes will be provided separately.
- Winners will be selected by a random drawing and announced on September 27, 2024.
- Winners will be notified by email and/or phone. If a winner does not respond within 7 days, an alternate winner may be selected.
5. General Conditions
- This Contest is in no way sponsored, endorsed, administered by, or associated with Instagram, Facebook, or any other third-party platform.
- By participating, you agree to release Instagram and Facebook from any liability related to this Contest.
- All entries become the property of Launch Physical Therapy & Sports Performance Center and may be used for marketing purposes.
6. Disqualification
- Any attempt to tamper with the Contest entry process or increase chances of winning through fraudulent means will result in disqualification.
- Launch Physical Therapy & Sports Performance Center reserves the right to disqualify any participant at its sole discretion.
7. Privacy
- By entering, participants agree to share their contact information with Launch Physical Therapy & Sports Performance Center for purposes of administering the Contest.
- Personal information will not be shared or sold to third parties without prior consent.
8. Limitation of Liability
- Launch Physical Therapy & Sports Performance Center is not responsible for lost, late, misdirected, incomplete, or inaccurate entries, or any technical issues that may affect participation.
- By entering, participants agree to hold harmless Launch Physical Therapy & Sports Performance Center from any and all liability, injury, loss, or damage that may result from participation in the Contest or from the acceptance, receipt, possession, or use of any prize.
9. Modification or Cancellation
- Launch Physical Therapy & Sports Performance Center reserves the right to modify or cancel the Contest at any time without notice due to unforeseen circumstances.
