David Herzberg

Doctor of Physical Therapy

Originally from Austin, Texas, Dr. David Herzberg (Dr. Dave), moved to Phoenix, Arizona in order to pursue his education in the healthcare industry. A lifetime athlete with a passion for fitness and endurance sports, Dave realized his interest in pursuing a career as a physical therapist. Throughout his undergraduate studies at Arizona State University, he interned as a Student Athletic Trainer. Dave graduated Magna Cum Laude from ASU (Go Sun Devils!) with a degree in Exercise Science/Kinesiology. Upon receiving his certificate as an athletic trainer (ATC), Dave worked in local outpatient physical therapy clinics focusing on sports medicine and orthopedic rehabilitation. In 2000, Dave attended Northern Arizona University (NAU) where he received a doctorate in physical therapy (DPT).

For the past 17 years, Dr. Dave has specialized in sports medicine-related orthopedic and musculoskeletal injuries. Dr. Dave’s patients appreciate his customized health and wellness programs which are designed to support long-term function and prevention of future injuries.

A lifetime learner, Dr. Dave has obtained certification in several evidence-based practice treatment models:

  • - Strength and Conditioning (CSCS)
  • - Integrative Dry Needling (CIDN)
  • - Augmented Soft Tissue Mobilization
  • - Techniques (ASTYM/IASTM)
  • - Manual Therapy (MTC)
  • - Cervicogenic dizziness, balance disorders, and TMJ dysfunction

Dr. Dave’s inspiration comes from his beautiful wife, Dana, who is a devoted educator, entrepreneur, and loving counterpart. Dr. Dave and Dana have a wonderful teenage son, Nathan, who is a natural athlete with a zest for both collegiate and professional sports. Together, we all enjoy fitness, traveling, attending sporting events, hiking and spending time with our menagerie of pets!

Zach Biesinger

Zach received his Doctor of Physical Therapy from the University of Utah. He is also orthopedic physical therapy residency trained from A.T. Still University and is a Board-Certified Clinical Specialist in Orthopedic Physical Therapy. His practice focuses on various musculoskeletal conditions of the neck, thoracic and lumbar spine, SI joint, hips, knees, foot/ankle, shoulder, and elbow/forearm. Though he treats multiple patient populations, he is particularly interested in treating weight lifters and high school and college athletes of all kinds, including football, basketball, volleyball, soccer, track, etc. He also enjoys treating general weekend warriors with shoulder and knee pain. With a background in personal training combined with physical therapy, he is equipped to help people transition from pain to performance and meet their lifelong fitness and activity goals.

Zach helps people improve their pain and function using a hands-on approach to teach correct techniques and the best exercises for their condition. He helps modify movement mechanics, enabling them to continue their sport/activity as much as possible while rehabilitating from injury. He’s also trained to use manual therapy techniques, including dry needling, joint mobilizations/manipulations, kinesio taping, soft tissue mobilization, and Instrument-Assisted Soft Tissue Mobilization (IASTM). During his free time, Zach enjoys competing in Spartan races, running, lifting weights, watching movies and football, and spending time with his family!

Jessie Hernandez

Jessie is a recent University of South Florida graduate and is working towards pursuing a career in physical therapy with an interest in pelvic floor therapy. As an essential addition to the Launch PT family, she focuses her time on enhancing and running our social media accounts while helping out with marketing and other aspects of the business.

Jessie spends her time making digital illustrations and creating products for her small business. She devotes her free time to volunteering for several nonprofit organizations including Infinite Wishes Foundation. She believes that the relationship you share with your community says a lot about you as a person and is passionate about leading the profession forward.

Kim Hogan

Human Resource Specialist

Kim relocated to the valley in 1998 with her family from Philadelphia, PA. Since 1996, she has worked in the human resources arena in various roles serving manufacturing, healthcare, and most recently education industries, helping businesses achieve their goals.

As an experienced Human Resources Specialist, Kim joined Launch Physical Therapy & Sports Performance Center in November 2022 and has been a wonderful addition to our family helping build upon our core values and providing assurance that our staff are content and thrive within our workplace. A wonderful mother of four children and two fur babies, Kim combines her talent for business and experience in resource management with her love of instilling a positive and productive culture in order to keep our clinic running smoothly. We could not do this without her!

Kim is an avid reader, enjoys paddleboarding at our local lakes, and loves to travel - her favorite destination is the beach, basically anywhere, and hiking with her husband and her Goldendoodle and Sheepadoodle. However, her greatest joy is spending time with her expanding family in her new role as Grandma, “Gigi."
